Information on the HIPAA-secure form is directly sent to our electronic health records system (Simple Practice) and does not utilize e-mail or our website's messaging system (which directs messages to us via e-mail).

Please do not include sensitive personal data or information you wish to remain confidential in emails and/or general inquiry website contact forms.

While we use HIPAA-secure e-mail, messages sent using e-mail and the website's general message system (also found on the Contact page) are not encrypted and inherently not secure, meaning there is the remote potential of messages being intercepted by third parties.
